Thank you for your interest in our program. The first phase is called the Pre-
Application process. During this phase, we will pull a FREE TransUnion
credit report, which will allow us to view a snapshot of your credit history.

Please print the following forms to fill out and return by mail or fax to the
appropriate office:

Are you interested in running a full credit-report from the BIG 3? If the
answer is YES, you can request a FREE credit report from Equifax, Experian
& TransUnion. Click the links below for a mail-in request or submit your

Please note: you have the right to a free credit report, once every 12 months,
from each of the nationwide consumer credit reporting companies as stated
above.

If you qualify, you will enter the second phase of applying for a USDA-Rural
Development 502 Direct Loan.   We will send you a user name and
password so you can obtain the forms at your own convenience.
